then there is a list of attached documents;
last line: signature of the developer, date and stamp.

As an aid in filling out, you can use the information contained in the building permit - since it was filled out by professionals, there is no doubt about the correctness of the information specified in it.

The structure of the notification looks like this:

The form should be accompanied by documents confirming the completion of the work and the elimination of violations made during the construction (if any). A notification form on the elimination of violations during construction can be downloaded in the annexes to the article.

After the notification is received, the supervisory authorities carry out a final check, the result of which is an act of acceptance of the constructed object. On its basis, the developer can apply for the issuance of an opinion, which will indicate that the erected object complies with the current legislation.

I. GENERAL PROVISIONS

1. This Procedure establishes the requirements for federal executive bodies authorized to exercise federal state construction supervision, other federal executive bodies entrusted with state construction supervision by decrees of the President of the Russian Federation, the State Atomic Energy Corporation Rosatom and executive bodies authorities of the constituent entities of the Russian Federation, authorized to carry out regional state construction supervision (hereinafter referred to as the state construction supervision body) of inspections of compliance of works performed during the construction and reconstruction of capital construction facilities with the requirements of technical regulations, other regulatory legal acts and project documentation, including the requirements of energy efficiency and requirements for equipping buildings, structures, structures with metering devices for used energy resources (hereinafter - check rki), and the registration of the results of such inspections, as well as the requirements for issuing conclusions on the compliance of the constructed, reconstructed capital construction facility with the requirements of technical regulations, other regulatory legal acts and project documentation, including the requirements for energy efficiency and the requirements for equipping a capital construction facility with instruments accounting for the energy resources used or making a decision to refuse to issue such an opinion.

2.1. State construction supervision is carried out in the cases specified in Part 1 of Article 54 of the Town Planning Code of the Russian Federation.

2.2. 3. Grounds for conducting inspections:

a) receipt from the developer, technical customer of a notice of the start of construction, reconstruction of a capital construction facility, sent in accordance with part 5 of Article 52 of the Town Planning Code of the Russian Federation, of the completion of construction, reconstruction of a capital construction facility;

b) an inspection program developed by the state construction supervision body;

c) receipt from the developer, technical customer, the person carrying out the construction, a notice of changes in the completion date of work subject to verification, if the completion date of such work does not coincide with the time specified in the verification program;

d) receiving from the developer, technical customer, the person carrying out the construction, notifications about cases of emergencies at the capital construction facility;

e) receiving from the developer, technical customer, the person carrying out the construction, notifications on the elimination of violations, as well as the expiration of the period for execution by a legal entity, individual entrepreneur of an order issued by the state construction supervision body to eliminate the revealed violation of mandatory requirements;

f) the presence of an order (instruction) of the head (deputy head) of the state construction supervision body, issued in accordance with the instruction of the President of the Russian Federation or the Government of the Russian Federation or on the basis of the request of the prosecutor to conduct an unscheduled inspection as part of the supervision of the implementation of laws on the materials received by the prosecutor's office and appeals;

g) applications and applications of citizens, including individual entrepreneurs, legal entities, information from public authorities (officials of the state supervision), local authorities, from the media about the facts of the accident, violations of technical regulations, other regulatory legal acts and project documentation when performing work in the process of construction, reconstruction of a capital construction facility, including violations of mandatory requirements for building materials used, if such violations pose a threat of harm to life, health of people, environment, state security, property of individuals and legal entities, state or municipal property, or have caused such harm;

h) applications and applications of citizens, including individual entrepreneurs, legal entities, information from public authorities (officials of the state supervision body), local government bodies, from the media on the facts of attracting funds from citizens for shared construction apartment buildings and (or) other real estate objects in violation of the legislation of the Russian Federation on participation in shared construction apartment buildings and (or) other real estate objects.

II. PREPARATION FOR CONDUCTING INSPECTIONS IN THE PERFORMANCE OF STATE CONSTRUCTION SUPERVISION

4. The official (officials) responsible (responsible) for the implementation of state construction supervision at a specific capital construction facility shall be appointed (appointed) by order (order) of the head (deputy head) of the state construction supervision body (hereinafter - the official of the state construction supervision body ) (Appendix No. to this Procedure).

State construction supervision is carried out in the form of inspections.

Checks are carried out official the state construction supervision body, authorized on the basis of the relevant order (instruction) of the head (deputy head) of the state construction supervision body, issued in accordance with the requirements of the Federal Law of December 26, 2008 No. 294-FZ.

d) the approximate time spent by an official of the state construction supervision body to conduct each inspection;

e) documents to be submitted during inspections provided for by the inspection program;

f) officials or employees of the developer, technical customer or person carrying out the construction, the presence of which during the inspections provided for by the inspection program is mandatory.

In exceptional cases related to the need to conduct and investigate, on the basis of motivated proposals by an official of the state construction supervision body conducting the inspection, the inspection period may be extended in the manner and for the period specified in Part 3 of Article 13 of the Federal Law of December 26, 2008 . № 294-ФЗ.

15. The act drawn up based on the results of the inspection and the order issued on its basis shall be drawn up in 2 copies. The act of inspection shall be accompanied by materials and documents drawn up or received in the course of the inspection, or their copies. The first copies of the act and prescription, as well as copies of these documents, are sent (handed over) to the developer, technical customer (if there is a technical customer), the person carrying out construction on the basis of an agreement with the developer or technical customer. The second copies of the inspection report and instructions, as well as documents drawn up or obtained in the course of the inspection, remain in the file of the state construction supervision body.

In the absence of a manager, another official or an authorized representative of a legal entity, an individual entrepreneur, his authorized representative, as well as in case of refusal of the inspected person to give a receipt for familiarization or refusal to familiarize with the inspection act, the act is sent by registered mail with a receipt acknowledgment, which is attached to a copy of the inspection report stored in the file of the state construction supervision body.

16. After elimination of violations identified by the state construction supervision body, the developer, technical customer (if there is a technical customer), the person carrying out the construction, who committed the violations and to whom the instructions were issued, send to the state construction supervision body a notice of elimination of violations during construction, reconstruction of the capital construction object (Appendix No. to this Procedure).

After the expiration of the period for eliminating the identified violations specified in the order, the state construction supervision body in the prescribed manner conducts unscheduled inspection to monitor the implementation of a previously issued order.

In accordance with part 6 of article 52 of the Urban Planning Code of the Russian Federation, the person carrying out the construction (the developer or technical customer or the person carrying out the construction on the basis of the contract) is obliged to ensure the elimination of the identified violations and not to proceed with the continuation of the work until the identified deficiencies are eliminated.
